Duval County Implements New Statewide Vote-by-Mail Written Request Form

JACKSONVILLE – The Duval County Supervisor of Elections Office began offering the new Statewide Vote-by-Mail (VBM) Written Request Form on April 17th to comply with a requirement by the Florida Division of Elections that all written requests for vote-by-mail ballots be made using the new form.

The new DS-DE 160 form is intended to make it easier for voters throughout the state to uniformly obtain vote-by-mail ballots for future elections.

In addition to making a written request for vote-by-mail ballots using the new form, voters can request vote-by-mail ballots by phone, on our website, or by visiting our office at 105 East Monroe Street.

Requests must be made by the voter or, if designated by the voter, a member of the voter’s immediate family or the voter’s legal guardian.

Voters can request a vote-by-mail ballot for all elections through the end of 2024 including the Primary Election on August 20th and the General Election on November 5 th, or indicate their request is for a specific election.
