How to Correct a Signature on Your Vote-By-Mail Ballot or Provisional Ballot

If you returned your mail (absentee) ballot to the Supervisor of Elections, but forgot to sign the ballot or there is an issue with your signature, your ballot will not count unless you complete and return the Signature Affidavit for Vote-By-Mail Ballot  (DS-DE 139), no later than 5:00 p.m. on the 2nd day after the election. A Signature Affidavit is also available for provisional ballots. Please follow the instructions on the form carefully, as failure to do so may cause your ballot not to count.
